I don’t know anything about Fentanyl patches but with regard to Gabapentin, switching to Pregablin might help. I have had two discectomies on same disc and have bad lumber pain. Last year I tried Gabapentin but it did nothing other that put me in the land of the fairies 🙂
I recently started on Pregablin, same side effects but I do get some relief, not all the time but some is better than nothing. It’s the same type of medication (anti epileptic) but supposed to work better at lower doses than Gabapentin.
